Bottle Blows Up

Before anyone jumps to conclusions perhaps its best to find out what actually failed and how.

Recently some people have been advocating more strict testing and regulation on regs.
Also there is the thorny issue of properly torquing and gluing bottles on to regs (regs are glued on to bottles nowadays, because unscrewing the reg even an unnoticable amount can reduce the shearing resistance of the threads by a huge amount. Greatly increasing the risk of failure at full pressure)

These kinds of things are easy to gloss over. (ive never glued my bottle to my reg and ive always been allright) But perhaps its time to really tackle safety issues. Not start a witch hunt.

Guys,

I think a little bit of patience is required at this point, the first thing is it sounds like Nigel has been very lucky and thank god for that.
But if the bottle did explode then it only did it with some sort of combustible contamination inside the bottle.
It sounds (and it will be checked to confirm this) oil could be the culprit.
I don't know Nige personally but I do know he is not a fool so I can not see him doing anything stupid.
There are many different reasons how the contamination could have made its way into the bottle all of them preventable and some of them with a degree of liability from third parties.
I am sure the type/make of bottle (in this case) have no impact on the matter, in fact it could be claimed another type of bottle could have fully exploded.
Lets find out the facts before we all cause a fury for no reason.

Good luck Nige

But the bottle was fine, a slight burn on it, it was the fill nipple, someone had put oil on a previous bottle, once nige put his in the thing, filled up to around 5k, somehow, and then it blew, the fill nipple came CLEAN off, Plus the threads of the reg where in the gun, No idea where the body of the reg went! The bottle cover was burnt, the gun was burnt, and you could smell oil on the bottle, it wasnt his fault, it would've been the people before.
Scary thing is, I nearly went up with him! >.<!
If its the case that someone has put oil in / on their reg and its transfered to someone else's bottle nipple on the next fill then that needs to be properly investigated! If that could cause the explosion that just happened then it could effectively happen to anyone regardless of what bottle they use.

If you know what happened you need to be speaking to Kitch about this.

Awful news, I hope you recover quickly Nige!

I think some more care needs to be taken around such high pressures... things like this should never happen; people need to be aware of the dangers they can pose to others and themselves...

Nexus were training there today and some of the guys saw the whole thing happen. It looked like right before the bottle exploded there was a load of smoke coming from the bottle pretty much a copy of what happened to us when we borrowed a Stakko tank at a training in Buddapest with the bullets.

none of the guys there were idiots and would have used oil anywhere near the fill rig or in our tanks and people have already said on this thread that neither would Nige.

We have personally seen two Stakkos go up in flames now and there was of course the other Stakko in Madrid that went bang. People who are in the know about this sort of stuff have suggested that the inner plastic liner of the bottle could have something to do with this, Stakko are the only guys that use this and I personally have only known problems with these tanks but this may not be the case of course.

So how much do people need to witness and hear about before you guys wise up and stop taking risks. Of course nothing has been proved but after having lost my hearing for a while after the Buddapest "incident" and seeing it happen for a second time today it's too many times for me.

Until anything has been proven I would suggest that everyone fills their tanks with the bottle facing downwards towards the floor just in case and on a personal note, if you ever see any Nexus guys at a fill station and you have a Stakko tank, can you please wait until we have gone as I love these guys the way they are !!!

thanks and best wishes to Nige.
